&lt;a href="http://www.longwoods.com/content/22432"&gt;Mr. Ronson in “LHINS at Five Years – What Now?”&lt;/a&gt; addresses a topic that requires urgent, candid and constructive debate. This essay continues the discussion on a way forward.

First, the context: Ontario is a geographically vast province with greathttp://www.blogger.com/img/blank.gif chttp://www.blogger.com/img/blank.gifultural diversity. If Ontario were a country, it would be the 29th largest country in the world. Two of Spain or France and three Germanys could fit within Ontario’s borders. Few would suggest a single health system structure for Spain or Germany.

It is time to challenge the premise that one structure will fit all of Ontario. A few regions have very clear referral patterns, with primary and secondary clinicians referring to a tertiary/quaternary centre in the same region. Splitting the province into “integrated healthcare organizations” (IHO)s which are responsible for hospitals CCACs, primary care, ambulance services, public health and community care may work for this type of population where there are medium sized urban centers with defined referral populations.

In the GTA people generally do not live and access care within small geographical boundaries. Hundreds of thousands of GTA residents live in one community, and work and access most health care services in another community. Equally impactful is that residents living in a community travel to another to obtain health services from a culturally and linguistically compatible provider.

Choice of where and when to access services is a key outcome that is required in the future system. The goal is to respond to local variation in health needs, culture and population density while acknowledging variable access to clinicians, and ensuring provincial standards for outcomes and access are met. The challenge is greater equity for all: whether the most northerly or the urban.

So, how would I organize the province? I would say that the historic changes to the structure of Ontario’s health system have been based on the premise that form follows function. It has not worked. Instead of “form follows function” let us evolve to “form follows (desired) outcomes”. Rather than thinking first of the functions we want and creating silos around those functions, let us transcend this traditional approach and consider the outcomes that will lead to a healthier and more productive population. Then organizations can be held responsible for delivering those outcomes. An example of this is the Cancer Care Ontario (CCO) model. The CCO approach does not rely upon a single corporate structure, but works within multiple corporate structures defining clear goals, accountability and outcomes.

I have seen no evidence from other jurisdictions that firing organizational boards and implementing a regional model have achieved long-term success in producing the desired outcomes. In fact, it appears that every time providers in the healthcare system take their eye off their primary foci – providing patient care – the system becomes less oriented on patient care and efficiency. Most other provinces appear to be changing their regionalization model every 5 years or so, creating 2 years of havoc, 2 years of realizing the model does not work and one year of planning for the next change. If we want to enter that cycle, we can begin tinkering with structures.

To date, Ontario has chosen to observe this behavior and has in effect been a Canadian control group for Provincial management of healthcare and its impact on efficiency. Ontario results of note: average lengths of stay in hospitals that are consistently so low that national length of stay benchmarks are not used in Ontario because they are irrelevant; an aging at home strategy in Toronto Central LHIN that leads the nation.

My opinion is that many historic structural changes are wasteful and unnecessary. It is time to change this debate and focus on outcomes. We need to identify what system OUTCOMES are desired by , and then hold organizations jointly accountable for the outcomes. The structures used to deliver on the outcomes may vary greatly across the province. But the focus would be on equity of outcome, not consistency of corporate structures and models.

Who, then, should be part of this discussion: primary care, secondary care, tertiary care, quaternary care, community care, laboratories, and public health. If we were to amalgamate these current structures into a mega-organization there would likely be considerable and unnecessary cost, strife and political unrest. Alternately, we could choose to define specific outcomes and hold each of the provider groups/organizations jointly responsible and accountable for delivering on those outcomes.

We should differentiate populations and their outcomes and assign lead accountability for these desired outcomes to specific groups/organizations. For example, the lead accountability for frail seniors and chronic disease management should not be the acute care sector, but should be primary care providers or CCACs. This model requires thinking of different populations and determining which players would provide the most effective and appropriate care to that population. Authority and accountability would then rest with that group/organization to develop the structures and partnerships to achieve the desired outcomes.

As an example, a small rural hospital staffed primarily by family physicians belonging to a family health team would be accountable, jointly with the family health team, and the CCAC for outcomes related to access and quality of primary and secondary care. These outcomes would be monitored by a regionally designed structure that would hold individuals and organizations accountable for achieving the desired outcomes. That body needs to have the authority and responsibility to make it happen. Does that mean we have to demolish everything to achieve this outcome? No. It means that roles, responsibilities and authorities need to be changed. Alignment of acute care, CCAC, LHIN, ambulance, public health, and healthcare oversight with municipal boundaries would help.

Without overstating their significance, I am hopeful that my musings may prompt consideration of a perspective different to that in other provinces. In a time of unrest, political uncertainty, there is often a tendency to latch on to simple solutions that are well intentioned but not necessarily well thought out. Change is costly. Why would we waste funds duplicating and standardizing regionalization models that others have discovered do not work, and deliver inadequate health care in the process? We need to have a thoughtful yet quick change that is focused on the desired outcomes, and holds individuals and organizations accountable for the achievement of those outcomes. How? &lt;p&gt;There are numerous reasons for maintaining a food diary. It is not exclusively about diet. While weight loss could be a valid reason for using a food diary, other individuals may want to actually gain weight or simply know sugar intake, calories or simply track carbs. In preparation for this vendors of healthcare software come together to demonstrate how interoperability is achieved. The sessions are informative and very useful, but at times incomprehensible as acronyms fly about. &lt;br /&gt;

We have EMR, EHR, PHR, PHP, HIAL, DICOM LIS, DIS, HIS, XDS; have I missed any? Words have meaning, the issue I have with excessive use of acronyms is that meaning is too often lost. Someone with a product will tag it with an acronym that doesn't apply. Who confirms that the label matches the acronym?

Our products are patient assessment tools, not EHRs nor EMRS; because these have significant definitions and connotations. We are not a CRM or ERP. &lt;br /&gt;


Our Clarity Health Journal is not a PHP or PHR it is a consumer health application. Other folks have created a category of consumer health platform. Can you identify a platform from an application? I run a web app using google chrome, does this make chrome a platform? no it is a browser. When we use semantics and jargon to describe something we distance our self. Only similar initiates can understand us, or in reality believe that they understand us.&lt;br /&gt;


Interoperability is all about systems communicating with one another seamlessly. Software that can share information from multiple sources. Data exchanged between pieces of the health care environment, a hospital record with doctors' office charts. The radiologist seeing the information from different sources, labs, drugs and imaging, perhaps over long distances, from technology sources that are unknown to them. It is a process I have gone through countless times over the past twenty years, numerous proposals submitted to meet the requirements of clients who have posted a Request For Proposal (RFP). I've learned a great deal in that time period.

Recently, I was involved in writing an RFP, helping a client to solicit solutions. Building an RFP is challenging. First you must be fair giving all proponents the same information and equal chance to respond. Second you must balance the requirements of the client while also giving leeway to the proponents to present innovative solutions. This is difficult at times.

Having worked on so many proposals you learn to read the signs within the RFP writing. Evaluating and determining if you should respond to a RFP is the first decision a company must make. The chances of winning a proposal is only one factor. There is the determination of the cost to response versus the value of the win. Time is another element to consider, having the resources to respond is as important as having the resource to deliver on the project should you win the bid.

Through out the process you hope that the writer of the and issuer of the RFP will give you a chance to show what you have. A proposal is words on paper, even pictures can only tell 1000 words. The advantage we have as english speaking professional is significant. English is the most popular foreign language, and while it may not be the language spoken by the most people, it is definitely the world’s largest "second" language and by far the first language of the internet. 

There may be a growing number of international websites in mandarin, arabic, spanish and portuguese many of the established sites cater to english speaking web users. Translation and geographic preferences for google, yahoo, facebook and other international sites is helpful for international users. Still many web users know or learn english to improve on the web experience.

I find automatic translation of websites to be spotty at best. Some phraseology is not translatable, and words are translated with literal meaning that can be inaccurate. 

Knowledge is wrapped in language and leads to better understanding. While the global language of the internet is English the future of the internet is its diversity and global reach in peoples native tongue.

Canada has a large multicultural population the we can use to advantage with web and software development internationally. The fabric of our country is strengthened by its diversity. This is also a tremendous asset. Canadian companies working in the technology field can leverage our national diversity to expand into foreign makes more easily by hiring professional with diverse languages and cultural knowledge right here in Canada, while foreign companies need to outsource such translation services. Canadian companies should and must look globally for greater advantage using Canadians. Clarke'/><title type='text'>Clarity Health Solutions and the Integrated Assessment Record (IAR)</title><content type='html'>As most Healthcare professionals are aware, the &lt;em&gt;Community Care Information Solutions Group&lt;/em&gt; has been busy working on a provincial solution to manage and maintain the &lt;em&gt;&lt;strong&gt;Integrated Assessment Record&lt;/strong&gt;&lt;/em&gt;.&lt;br/&gt;&lt;br/&gt;

The purpose of the IAR, as it is called, is an initiative within CCIM to allow assessment information to be viewed by health service providers in a secure manner.  It can be used to access and view assessment information which is centrally located at a Health Information Network Provider (HINP) identified by the LHIN Steering Committee.&lt;br/&gt;&lt;br/&gt;

The IAR repository, being a central repository of collected data, is a means to share assessments across Community Mental Health organizations, Hospital Emergency departments, Primary-Care and Hospital Mental Health in-patient teams.  This is done in order to provide &lt;em&gt;improved quality care&lt;/em&gt; ...&lt;br/&gt;&lt;br/&gt;

Consulting Cadre was one of the first vendors to jump on to the idea of the IAR repository by working with the IAR team and integrating the necessary behaviour and requirements into the &lt;em&gt;Clarity Healthcare Solutions&lt;/em&gt; (CHS) family of products - CCRS, NRRS, MHRS and HCRS.&lt;br/&gt;&lt;br/&gt;

At the time of writing this entry, the IAR team is concentrating only on the sharing of MHRS assessments.  CHS has been modified to adhere to the IAR team's requirements , has been unit-tested by CHS developers, and is currently ready and waiting for general Vendor Acceptance Testing to begin in October with the IAR team.  Once Consulting Cadre passes this acceptance testing - the CHS-IAR solution can and will be rolled out to our customers in the effected LHINs for deployment.&lt;br/&gt;&lt;br/&gt;

There has been discussion with the IAR team that this level of provincial shared assessment repositories will be potentially utlitized in the future for other Healthcare domains such as CCRS, NRRS and HCRS ...&lt;br/&gt;&lt;br/&gt;

Today we look back at the last 15-20 years with amazement at the changes we are witnessing. Time marches on and brings changes some foreseen while others aren't. 

In university I took a "Future Studies" course. It was intriguing and enlightening. Did you know you can be a "futurist"? Simply join the&lt;a href="http://www.wfs.org/index.html"&gt; World Future Society&lt;/a&gt;. Knowing about the future involves both an understanding of the past and the integration of the present. Everyone should study the future, after all it is we will be living the rest of our lives there. 

Flying cars, and trips to space were all promises of the future when I was a teenager. It did happen or at least not yet. We have seen a great deal of other developments, the internet, global communication networks are just a few. 

I also recall as a child thinking about having a computer companion. A know-it-all machine that I could ask anything and get an answer. Some may say its not quite there yet, but I believe we are very close to it.

Our conversations primarily related to the challenges she was facing.  While the specifics were unique to her and her organization, every one of her conundrums were ones which I had dealt with myself. Innovation in healthcare being exported to the US market will also benefit Canadian Healthcare. Clarke'/><title type='text'>CIHI 2010 Changes Coming ...</title><content type='html'>Just thought that I would drop in a little note to all to let you know that we are currently working on implementing the mandated CIHI (2010) changes to the Clarity Healthcare CCRS, MHRS and NRRS modules.

This year, CIHI has made some significant changes to all of the modules.  These changes include 

- new Assessment fields
- removed Assessment fields
- updated Edit Rules (as always) 
- and most importantly this year - changes to the &lt;em&gt;Outcome Measures&lt;/em&gt; in the modules 
&lt;br /&gt;

The changes will be tested shortly with CIHI (for our vendor testing cycle) and once that is complete, you can begin to schedule your &lt;em&gt;Test Server&lt;/em&gt; upgrades at your Facilities - so that you can review the changes for yourself.

And in order to make this user experience even more portable, we are developing CHJ apps for the iPhone. These apps will allow a busy, on the road user to make self-monitoring entries for things like blood glucose without connecting to the CHJ, and commit their offline entries to the CHJ at their convenience. The app will also provide a web viewer for connecting to the CHJ proper, and server-side web views which are tailored for the resolution of the iPhone screen. The goal is to provide CHJ users with the convenience required to maintain an accurate and up-to-date personal health journal.&lt;div&gt;

Many of the participants at HIMSS are US hospitals and agencies. Some attendants come from other countries. And of course a number of Canadian hospital and health agency representatives will also attend this international trade show. 

Canadians appear to look south to the US as a source of expertise. Even with our Canadian pride regarding universal healthcare we look to learn from the experiences of the US healthcare process. There are many resources and educational session. Tremendous opportunities to meet healthcare leaders.

The main topics this year will be the concept of "meaningful use" in EHR/EMR deployment. The US is looking to invest $34 Billion to modernize their healthcare system. Standards and outcomes are central to the ARRA funding. 

While in Canada we switched to &lt;a href="http://www.acep.org/practres.aspx?id=30476"&gt;ICD10&lt;/a&gt; in 2004/05 for health records diagnoses coding, the US is still using ICD9. Canadian companies can help with this transition. International standards are being adopted in Canada such as &lt;a href="http://www.ihtsdo.org/snomed-ct/"&gt;SNOMED-CT&lt;/a&gt; and HL7 V.3.0 data exchange and messaging, we can provide help with Health Information Exchanges(HIE) roll outs in many states. HIMSS is a great venue to present and share experiences. 

For us the &lt;a href="http://clarityhealthjournal.com"&gt;Clarity Health Journal&lt;/a&gt; is the main reason for attending HIMSS. We see the personal health application as pivotal to meaningful use. Empowering patients and their families to be participant in their healthcare system.

We may need the occasional check-up, flu shot or prescription. Our family doctor can schedule an annual physical and for the other common complains see you for 15-20 minutes. 

In consulting we are all aware of the 80-20 rule or Pareto principal. It goes like this, 20% of the people usually result in 80% of the costs. It is a general rule that seems to pop-up everywhere. And it is no surprise that it is the case in health care.

High cost of care endangers our health care system. We can apply our attention to 80% of the population and help deliver the low cost health care more efficiency. Or we can apply our attention to the 20% or the population that drive the majority of the high-cost of health care.

The value of eHealth is still unproved, yet the potential benefits is significant. Using electronic health records and online health systems means greater integration of care and data. Quality improvements can be determined once data is collected and analyzed. The information and knowledge can be used to improve outcomes and reduce costs.

What does this mean for the majority of us? Well we can visit any doctor's clinic and they can have access to our information. And when we reach an age when we require greater intervention in our health, historical data about our health will be available online. Greater access, better service and lower cost that is the promise of eHealth. Being creative is something we are taught. Being a visionary,I believe is something we acquire through experience. Which of these two is most valuable? 

Imagination is a trait we encourage in children. Ingenuity is not taught, but is part of our character. Invention is only possible when it is used to produce or create. Being creative and taking a vision to fruition is the greatest of accomplishments. 
 
Ideas without follow through are puffs of smoke. Taking an idea and transforming it to a product, a tool and invention takes a belief in our abilities. All of this is something we can learn and must strive to achieve.
